Clinical case orthognathic surgery

Our patient, a 27-year-old woman, came to Clínica Birbe concerned about the lack of harmony in her face: she had a receding jaw, a poorly projected chin, an open bite and a narrow upper jaw that caused her gums to show excessively when she smiled. After a detailed study with 3D scan, clinical photos and functional analysis, we designed a customized treatment that combined pre-surgical orthodontics, segmented bimaxillary orthognathic surgery and an advancement mentoplasty. The upper jaw was segmented by means of a Le Fort I osteotomy in several pieces, which allowed us to correct the open bite and widen the jaw in a precise manner; in addition, it was lightly impacted to reduce the gingival smile. The mandible was moved forward with counterclockwise rotation to restore good occlusion and correctly project the lower third of the face. Finally, we perform a bone mentoplasty to achieve a natural, feminine and proportionate profile. The entire procedure was digitally planned with 3D technology, which allowed us to offer maximum precision, predictability and safety. The result has been a functional and aesthetic transformation, with a balanced smile, a harmonious facial expression and, above all, a patient who finally recognizes and likes herself in front of the mirror.
