What Is Rapid Orthodontics?
Corticotomies are small cuts made in the gum and cortical bone that help the teeth move more quickly during orthodontic treatment. They can be performed using conventional orthodontics or Invisalign®. It is a painless and minimally invasive procedure.
With age and as the years go by, bone remodeling—which refers to the rate at which bone cells act to cause tooth movement—slows down.
In these cases, orthodontic treatment does not progress as quickly as we would like.
Corticotomy causes trauma to the cortical bone and the gum, which induces RAP (Regional Acceleratory Phenomenon), a transient phenomenon characterized by regional acceleration of bone remodeling (demineralization/remineralization of the bone).
They would also be indicated when we need to perform large dentoalveolar movements but the thickness or type of the patient’s gum tissue prevents us from doing so. By using corticotomies, we can prevent problems such as gum recession or dental dehiscence, which can occur when there is crowding or compression in patients with a very fine biotype.
In summary, orthodontic treatment using corticotomies is a good option when we want to achieve faster results or when we need to make large movements without causing side effects in the patient’s periodontium.
Treatment Process
The process begins with a careful evaluation: rapid orthodontic treatment is indicated only in selected cases involving mild problems localized in the front teeth. We take photographs, perform scans, and, in many cases, create a digital simulation of the result. If your case is a good candidate, we’ll determine the treatment plan (braces in the front teeth or aligners) and the timeline. Visits are scheduled every 3–4 weeks, since activation occurs more frequently. At the end of treatment, we remove the braces and provide retainers. If we discover during the evaluation that the case requires more extensive treatment, we discuss this honestly before moving forward.
Recovery time
There is no surgical treatment. After having braces or aligners fitted, you may experience mild pressure-related discomfort for 24–48 hours, which can be managed with mild pain relievers. Any chafing caused by braces can be treated with orthodontic wax. Oral hygiene requires special attention, including brushing after every meal and using an interdental brush. With aligners, the appliances are removed for eating and oral hygiene, and are worn for at least 22 hours a day. It is easy to adapt to and compatible with work and social life.
Expected Results
The results of properly indicated rapid orthodontic treatment are visible within the first few weeks. At the end of treatment, the anterior teeth are aligned, improving both aesthetics and function. Stability depends on the correct use of the retainer: without it, the teeth may tend to shift back into their original positions, especially if the original cause (crowding due to lack of space) has not been fully resolved. We document the results with comparative photographs and maintenance inspections included in the plan. The change in one’s smile is usually noticeable.
Duration of treatment
The typical duration of accelerated orthodontic treatment is 4 to 9 months. Visits are scheduled every 3–4 weeks. After removing the appliance, we provide a fixed lingual retainer and, in many cases, a night guard. Follow-up visits are scheduled at 6 and 12 months. It’s the fastest option when the case is truly compatible. If you are looking for more significant changes or comprehensive treatment of your entire bite, a complete orthodontic treatment plan is recommended; although it takes longer, it addresses the underlying cause.
