When a malocclusion cannot be solved with orthodontics alone,a more comprehensive treatment must be carried out in which orthognathic surgery and orthodontics come together to resolve the problem. The patient may suffer from a malposition of the maxillary bones, causing pathologies due to incorrect bites, pain in the TMJ, or breathing and swallowing problems, among others. 

Therefore, thanks to this combination, harmony is achieved in the jaw structure, along with  a major change in facial  aesthetics and  breathing  and, of course, the patient’s quality of life. 

This complete treatment is therefore ideal for patients suffering from pathologies such as retrognathia (backward position of the lower jaw), prognathism (too advanced position of the lower jaw), facial asymmetry (notable imbalance between the two sides of the face), obstructive sleep apnea syndrome, or an open bite (no contact between the upper and lower teeth). 

Pre-surgical vs. post-surgical orthodontics: when are they present?

  • Orthodontic treatment is carried out before the operation to decompensate the teeth position, that is placing the teeth in the ideal position in relation to its bony base, regardless of the occlusion, which will be corrected by the subsequent orthognathic surgery. . After a period of time, of around 6 to 8 months, the patient undergoes an operation to position the jaw bones correctly, and after surgery, the orthodontics continues to stabilize,  finishing aligning the teeth. In fact we can consider that orthodontic/orthognathic surgery treatment, as an orthodontic treatment in which at a certain point there will be an orthognathic surgery. 
  • In cases where we can perform the surgical treatment first and the orthodontic treatment after surgery, the term Surgery First approach is used. 
  • When this surgery is at a very early stage of the orthodontic treatment, we use the term “surgery early approach”. And when surgery is done 6-to 10 months after the start of orthodontics, then we use the term “conventional approach”. The decision to do a surgery first, surgery early or conventional approach is based on the orthodontic movements required to correct the problem. 
  • Beforehand, a study will be carried out to analyze the facial structure and decide which treatment is the most suitable to achieve the best results. Virtual Surgical Planning (VSP) allows us to perform the surgery virtually on the computer, analyze the planning and once the movements have been optimized, print surgical splints that allow us to faithfully reproduce the previously planned movements. You will also be able to see your surgery before performing it so that you will understand perfectly the need to perform certain orthognathic movements.

On the other hand, there are two different types of orthodontics used, pre-surgical and post-surgical. As their names indicate, the difference between them is the moment at which they are applied in relation to the surgery: before or after orthognathic surgery. The time at which treatment is carried out depends on the objectives to be achieved. 

Pre-surgical orthodontics aims to change the malposition and inclination of the teeth to position them correctly at the base of the bone. Post-surgical orthodontics, on the other hand, serves to maintain the teeth in the position that has been achieved after the intervention, to stabilize the bite and to align the teeth perfectly.

What steps are carried out in surgical orthodontics?

Diagnosis. Our doctors will analyze the patient's face, smile and type of malocclusion. Pre-surgical orthodontics. This treatment will decompensate the two dental arches, placing the teeth in the correct position to be able to carry out the intervention afterwards. Orthognathic surgery. Surgery is performed through the movement of the upper jaw , mandible and chin if needed. Always through the mouth, without the need to make any scars outside the face. Post-surgical orthodontics. After around two weeks of recovery, the patient will have to continue with orthodontics again, for a period that can last around 6 months (depending on each case) to make the final touches and achieve the perfect smile. In short, surgical orthodontics is that which is carried out in the context of orthognathic surgery. It is an orthodontic treatment in which a surgical intervention is carried out at a certain point in the treatment. At all times during the treatment, the patient wears orthodontics.

What is the postoperative period for pre-surgical orthodontics and orthognathic surgery like?

Depending on each case, the patient will have a longer or shorter recovery. Normally, it is estimated to take between one and two weeks. In any case, the patient must follow our instructions to avoid any type of problem that may arise and, in this way, facilitate the patient's recovery. For example, following a liquid diet and, after a few weeks, a soft diet; absolute rest or wearing elastic bands for containment.
