What is Blepharoplasty?

Blepharoplasty is the surgery that corrects the bags that may appear in the eyelids, which may be a consequence of muscle relaxation and with the passage of time the skin loses laxity and causes them. It is an eye rejuvenation surgery.


The maxillofacial surgeon, Dr Birbe, medical director of Clínica BIRBE performs this treatment. The surgery is performed under local anesthesia but we recommend intravenous sedation for greater relaxation and tranquility of the patient.


Women and men over 40 years of age, who show excess skin or fat on the eyelids but maintain a good facial structure. They are usually active people and therefore find it extremely annoying to wake up in the morning with a tired appearance that does not at all correspond to their good mood.

Types of Blepharoplasty

Lower blepharoplasty
The surgery is usually performed transconjunctivally, that is to say, an incision of approximately one centimeter is made inside the eyelid, without cutting the skin, to directly access the fat pads. Subsequently, the three compartments are searched and the bags are dried one by one, in the right quantity. Once these accumulations have been eliminated, the operation is completed by placing a stitch in the conjunctiva or leaving it unsutured, depending on the surgeon’s preference, since it is an area that closes very easily. Another technique is the transposition of fat pads. In other words, we move them towards the area of the dark circles under the eyes, in such a way that we blur the junction between the eyelid and the cheek, erasing the furrow that marks the dark circles under the eyes.

Upper Blepharoplasty
The excess skin is painted on the skin and the incisions are made at the limit of the natural crease of the upper eyelid, where the scar will be hidden. Next, the excess skin spindle and a thin band of orbicularis oculi muscle are removed to locate the fat pads and resect the necessary amount. The skin is carefully closed again, suturing small internal stitches that will maintain the tension. Finally, thin bandages are applied to protect the scar until the stitches are removed, usually between four and seven days after the operation.

Surgical Steps

In the first consultation, a thorough evaluation of your vision and general health is performed. In addition, your daily habits are also analyzed (whether you wear glasses or contact lenses, if you are a smoker, if you take any type of medication, possible allergies, etc.), as these are factors that influence the study. Dr Birbe will also provide you with some guidelines to follow during the weeks prior to the operation, in terms of diet and habits.

The surgery is performed in an operating room under local anesthesia or local anesthesia plus sedation, depending on each patient. A blepharoplasty procedure usually lasts approximately 1h 30 min for upper and lower eyelids and half an hour in case of upper eyelids.

Typically after eyelid surgery, 90% of patients are back to normal within a week. Recovery depends on different parameters of the eyelids as they evolve differently.
